Referrals

Referrals for assessment and treatment can be made by prospective clients (i.e., self-referral) or by their family physician or other service providers (for example occupational therapists, psychiatrists, physiotherapists or clients' legal counsel).

Individuals wishing to self-refer can contact me by telephone (905-988-9517) or email (dr.sbaker@yahoo.ca or follow the "Contact Me" link); please be aware, however, that email is not a secure means of communication. Please let me know your name, contact information (e.g., home or cell number, email address) and times when I can call you. Please also let me know whether I can leave a detailed message for you, or if you prefer I leave my name and phone number only (or no message at all).

Physicians and other service providers can fax referrals to 289-434-4760 or make referrals by telephone to 905-988-9517. Unless you have your client's written permission to communicate about them electronically over the internet, please do not send referrals by email.

Please be advised that, from time to time, I am required to close my practice to new referrals when my client levels are at capacity.
